What Documents Are Required

  1. Completed residence card application (filed by the wife)
  2. Wife’s passport — original and copies of all pages
  3. 4 recent passport-style photos
  4. Marriage certificate with apostille and sworn Polish translation
  5. Copy of the husband’s residence card or status document
  6. Proof of shared residence (rental agreement, registration)
  7. Proof of the family’s financial means (husband’s income statement)
  8. Health insurance for the wife

A marriage certificate issued in Ukraine requires an apostille before it can be translated and submitted. Without it, the document won’t be accepted at intake.

IMPORTANT: if the marriage was registered abroad and hasn’t been entered into the Polish civil registry, the Urząd may request additional confirmation or a transcript of the act. This isn’t a refusal — but it adds 4–8 weeks to the process. Better to check this before you file.

  • Shared utility bills or subscriptions

Common Mistakes When Applying Through a Spouse
